Service Coordinator (NDIS)

Key responsibilities

• Manage and maintain accurate participant documentation and records
• Monitor participant goals and implement support plans
• Conduct risk assessments and ensure safety compliance
• Coordinate rosters and ensure effective service delivery
• Liaise with participants, families and external stakeholders
• Ensure compliance with organisational policies and NDIS standards

Required profile

• Degree in Social Work
• Experience in disability, mental health, social work or community services (desired)
• Experience conducting risk assessments and managing incidents

Required skills

• Strong understanding of NDIS practices and documentation requirements
• Excellent communication and stakeholder engagement skills
• Ability to implement and monitor support plans
• Ability to lead, support and supervise a team
• Experience using systems such as Brevity
