Senior Pharmacist – Medicines Safety

About the role

The Senior Pharmacist – Medicines Safety will lead the medicines safety portfolio at Alfred Hospital, ensuring safe, effective, and financially responsible medication management across the organisation. Working closely with the Lead Pharmacist – Medicines Safety, you will drive strategic initiatives and day‑to‑day improvements to enhance patient outcomes.

Key responsibilities

• Manage and develop the medicines safety portfolio, including policy review and system optimisation.
• Monitor, analyse and improve medication safety processes across pharmacy and the wider hospital.
• Collaborate with Stewardship Pharmacists and the Digital Health team on risk‑management initiatives.
• Lead clinical activities such as therapeutic drug monitoring, partnered pharmacist medicine charting, nicotine replacement therapy and analgesia de‑escalation.
• Support education, research and advanced practice programs within the pharmacy department.

Required profile

• Registered pharmacist with extensive clinical experience in a hospital setting.
• Demonstrated expertise in medicines safety, clinical risk management and interdisciplinary collaboration.
• Strong commitment to continuous improvement, education and research.

Required skills

• Experience with electronic medical record systems, specifically Cerner.
• Familiarity with pharmacy dispensing platforms such as iPharmacy.
• Knowledge of automated medication storage solutions like Omnicell.
• Understanding of digital health tools applied to medication management.
