Registered Nurse – Hospital in the Home (Newcastle)

About the role

Join the Hospital in the Home (HITH) team to deliver hospital‑level care to patients in the comfort of their own homes. This permanent position offers autonomy, variety and the chance to make a real difference in patients' recovery while working within a supportive, multidisciplinary environment.

Key responsibilities

• Perform comprehensive nursing assessments and clinical observations.
• Administer and reconcile medications.
• Provide wound care and ongoing treatment management.
• Collect pathology specimens.
• Educate patients and carers on care plans.
• Liaise with medical officers, general practitioners and other healthcare providers.
• Monitor patient progress and escalate concerns when required.

Required profile

• Current registration as a Registered Nurse with AHPRA.
• Extensive clinical nursing experience.
• Strong assessment and clinical decision‑making abilities.
• Excellent communication and interpersonal skills.
• Ability to manage competing priorities and work autonomously.
• Confidence delivering care independently in patients' homes.
• Patient‑centred approach with strong advocacy skills.
• Quickly assess situations and respond to changing clinical needs.

Required skills

• Medication administration and reconciliation.
