Registered Nurse Grade 3A - District Nursing

About the role

St Vincent's Health Australia is recruiting a permanent part-time District Nurse (Grade 3A, 0.4 FTE) based in Fitzroy. The District Nurse provides high-quality nursing services to a diverse community, using a wellbeing and reablement approach to improve clients' health and independence and support carers. The position sits within the Care at Home division, which supports people to stay at home and receive clinical care there.

Key responsibilities

• Assess broader care needs and deliver nursing care within professional boundaries, covering assessment, care planning, treatment, review and education of clients and carers
• Promote active involvement of clients and carers in care planning
• Work cooperatively with local service providers and encourage coordinated case management, especially for clients with chronic and complex needs

Required profile

• Relevant clinical experience in the community setting and/or across a broad range of acute medical/surgical services
• Experience with clients who have multiple comorbidities and complex care needs
• Current nursing registration with AHPRAC and current driver's licence
• Current national criminal history check, or willingness to obtain one, and current NDIS Worker Screening Check
• Current valid Working with Children Check
• Willingness to meet mandatory immunisation requirements and the Victorian government COVID-19 vaccination direction

Required skills

• Highly effective interpersonal skills, including problem solving, conflict resolution, negotiation and teaching
• Excellent organisational, coordination and time management skills
