About the role
Join a regional team delivering Residential Aged Care Funding Assessments across Orana Far West NSW. You will travel to aged‑care homes, interview residents and staff, and review health records to complete AN‑ACC assessments that determine funding levels.
Key responsibilities
- Conduct AN‑ACC assessments using desktop documentation, observations and conversations.
- Travel across the designated region and provide support to other regions as needed.
- Maintain accurate records and submit assessments in line with Department of Health requirements.
Required profile
- 5+ years post‑registration clinical experience in aged‑care.
- Current unrestricted AHPRA registration (Nursing, Physiotherapy or Occupational Therapy).
- Demonstrated experience with people with dementia and conducting clinical assessments.
- Own vehicle with comprehensive insurance for regional travel.
Required skills
- Proficiency with the AN‑ACC assessment tool.
- Ability to engage older Australians and stakeholders in residential facilities.
