Physiotherapist (Transitional Aged Care Program)

About the role

St Vincent's Hospital Sydney is recruiting a Physiotherapist (Level 1-2) for its Transitional Aged Care Program (TACP) in Darlinghurst. This is a full-time, maximum-term contract until February 2027, working Monday to Friday, 8:30am to 4:30pm. The salary is $79,419 to $113,301 plus superannuation and salary packaging.

Key responsibilities

• Deliver physiotherapy assessments and evidence-based interventions to clients in the Transitional Aged Care Program within the community
• Provide comprehensive case management to help clients maximise their independence and reach their rehabilitation goals
• Collaborate with community services, multidisciplinary healthcare professionals, clients and their families to coordinate care
• Contribute to quality improvement initiatives that improve service delivery and client outcomes
• Provide clinical education, coaching and support to colleagues in the TACP and wider community-based services

Required profile

• Tertiary qualification in Physiotherapy and current registration with AHPRA
• Current NSW driver licence (Class C or above) and ability to travel throughout the community
• Ability to work autonomously and collaboratively in a multidisciplinary team
• Excellent communication and interpersonal skills with clients, families and healthcare professionals
• Commitment to clinical excellence through ongoing professional development
• Australian or New Zealand citizenship or an Australia visa granting work rights; visa sponsorship is not available
