Job description

About the role

This role supports people with disability, their families and carers across the Joondalup region, helping them navigate the NDIS, set goals and connect with community services.

Key responsibilities

  • Guide participants through NDIS applications and plan management.
  • Assist participants in mapping out personal goals and outcomes.
  • Link participants with local community groups, services and opportunities.
  • Promote community awareness of the NDIS and encourage involvement.
  • Track outcomes and meet key performance indicators.

Required profile

  • Approachable, positive and passionate about supporting people.
  • Strong listening and communication skills, able to build trust.
  • Relevant or lived experience supporting people with disability.
  • Knowledge of local community resources and services.
  • Ability to obtain Working with Children and/or Vulnerable People clearance.

Required skills

  • Proficiency with Microsoft Office 365.
