Enrolled Nurse Care Partner

About the role

Amplar Health, part of the Medibank Group, is looking for an Enrolled Nurse Care Partner to join its growing Support at Home service. The role works within a clinically-led model focused on prevention, early intervention and reablement, helping consumers stay independent and well in their own homes. You will build trusted relationships with consumers and their families, working alongside allied health professionals, nurses and the primary care network.

Key responsibilities

• Work closely with consumers, families and healthcare providers to deliver person-centred care that promotes independence, wellbeing and healthy ageing
• Undertake holistic assessments and develop tailored care plans
• Coordinate services that match each person's goals and aspirations
• Monitor wellbeing, spot opportunities for early intervention and help consumers take an active role in their health and lifestyle goals
• Collaborate with a multidisciplinary team to keep consumers connected, capable and thriving at home

Required profile

• Experienced Enrolled Nurse (AHPRA registered)
• At least 2 years' experience in aged care and community or home care
• Passion for person-centred care and building trusted relationships

Required skills

• Strong assessment, care planning and care coordination skills
• Confidence using digital systems and virtual care tools
• Excellent communication and clinical judgement
