Enrolled Nurse - Aged Care (Permanent Part-Time)

About the role

St Vincent's Care Services Hawthorn (locally known as Mary Mackillop Aged Care), a 91-bed facility in Hawthorn East, is looking for an Enrolled Nurse to help create a welcoming, professional and supportive care experience for its residents. The role is a permanent part-time position covering weeks 1 and 2 on Monday PM, Tuesday PM, Wednesday PM and Friday AM shifts.

Key responsibilities

• Coordinate person-centred care from admission through to discharge, working with the Registered Nurse on care planning
• Document a systematic, comprehensive and accurate assessment of residents and evaluate documentation on resident outcomes
• Maintain confidentiality and uphold residents' rights and dignity
• Implement interventions from the care plan and record progress, revising the plan as needed
• Facilitate communication between residents, families and medical, nursing, ancillary and allied health staff, and refer with the RN for comprehensive care
• Take part in clinical risk management, resident safety and continuous quality improvement
• Act as a preceptor for new Personal Care staff and carry out other reasonable duties directed by the Registered Nurse or Residential Care Services Manager

Required profile

• Current AHPRA registration as an Enrolled Nurse
• Diploma of Nursing or equivalent
• Knowledge of the Aged Care Quality Standards and person-centred care principles
• Medical administration competency
• An NDIS Worker Screening Check may be required
