Community Care Social Worker

About the role

Bayside Health, a public health service covering metropolitan Melbourne, the Mornington Peninsula, Koo Wee Rup, Bass Coast and Southern Gippsland, is hiring a Community Care Social Worker in Frankston. This is a fixed-term part-time position of 40 hours per fortnight until June 2027, classified as Social Worker Grade 2, Years 1 to 4. You will work with older people and those with complex health needs to help them live safely and well.

Key responsibilities

• Provide psychosocial assessment, care coordination, advocacy and case management.
• Support clients transitioning from hospital, helping them navigate services and access the right supports.
• Help clients maintain their independence and wellbeing.
• Work closely with residential aged care facilities, health professionals, community providers, clients and families.

Required profile

• Qualified Social Worker with eligibility of the AASW.
• Demonstrated experience completing comprehensive social work assessments.
• Experience working with at-risk and vulnerable people with complex needs in subacute/community health settings and aged care services.
• Advanced knowledge of social work theory such as crisis intervention and strengths-based approaches.

Required skills

• Well-developed written, verbal, interpersonal and computer literacy and communication skills.
• Ability to manage multiple competing tasks and prioritise effectively.
