Community Care Physiotherapist

About the role

Bayside Health is recruiting a Community Care Physiotherapist in Frankston, Victoria, as part of its Health Independence Program. The role works within the Wagner Chronic Care Model and coordinates intensive, short-term (3-6 months) outreach care and delivers allied health interventions within the physiotherapist's scope of practice to clients with chronic health conditions and complex healthcare needs, as well as providing post-acute physiotherapy for clients recently discharged from hospital. It is a Grade 2 position (Year 1 to Year 4 classification) working flexibly across acute and community-based services.

Key responsibilities

• Coordinate intensive, short-term (3-6 months) outreach care
• Deliver allied health interventions within clinical scope to clients with chronic conditions and complex needs
• Provide post-acute physiotherapy for clients recently discharged from hospital
• Facilitate collaborative, integrated care planning across acute and community services
• Support professionals and service users in navigating the health service system

Required profile

• Current Physiotherapist registration with AHPRA
• Demonstrated experience working independently and in a multidisciplinary team
• Demonstrated experience providing comprehensive home assessments and assessing equipment needs

Required skills

• Current Victorian driver's licence
• Ability to work flexibly and supportively in an ambulatory setting
