Clinical Pharmacist – Emergency Medicine (Grade 2)

About the role

Western Health is seeking a Grade 2 Clinical Pharmacist to join the Emergency Medicine Pharmacy team, providing specialist pharmacy support in the Emergency Department and Intensive Care Unit. The role focuses on medication safety, clinical decision‑making and optimisation of therapy for critically ill patients, while contributing to service improvement, education and research.

Key responsibilities

• Deliver clinical pharmacy services within the Emergency Department and Intensive Care Unit.
• Complete medication reconciliation, medication reviews and clinical interventions for high‑priority patients.
• Provide expert medicines information and evidence‑based recommendations to multidisciplinary teams.
• Monitor and optimise medication therapy to improve patient outcomes and reduce medication‑related harm.
• Participate in medication safety, antimicrobial stewardship, quality improvement and research activities.

Required profile

• Bachelor of Pharmacy or equivalent.
• Eligibility for general registration with AHPRA.
• At least three years' experience as a clinical pharmacist across a broad range of specialties.
• Strong clinical reasoning and problem‑solving abilities.
• Excellent communication and relationship‑building skills.
• Proven ability to work effectively within multidisciplinary healthcare teams.
• Commitment to evidence‑based practice, continuous improvement and professional development.

Required skills

• Experience using Electronic Medical Record systems such as Oracle Cerner.
