Clinical Nurse - Sub-Acute Transition Care

About the role

A Clinical Nurse is sought for a full-time position in the sub-acute Transition Care Service in Walkerville. The nurse provides clinical leadership and expert nursing care to patients with complex and ongoing healthcare needs, working within a multidisciplinary team to support clinical excellence and positive patient outcomes.

Key responsibilities

• Coordinate patient care and support smooth transitions across care settings
• Deliver high-quality, evidence-based nursing care
• Support governance, risk management and service coordination
• Contribute to operational processes and system improvement
• Promote consistent, safe and effective clinical practice across programs
• Take part in quality improvement, service development and multidisciplinary collaboration
• Provide clinical leadership and act as a resource for nursing teams

Required profile

• Registered Nurse with current unrestricted AHPRA registration
• Minimum 3 years of post-registration experience
• Experience or interest in governance, service coordination or operational management
• Current First Aid and CPR certificates
• Willingness to work a rotating roster
• South Australian Working with Children Check and National Police Clearance (or eligibility to obtain them)
• Willingness to provide proof of up-to-date vaccinations if successful
