Clinical Care Partner - Ageing Services

About the role

Footprints Community, a not-for-profit organisation with more than 35 years of experience supporting older Australians, is looking for a Clinical Care Partner in wider Brisbane. The role provides care management, administrative support and coordination of services for clients receiving individualised Home Care Packages, under a person-centred, consumer-directed model that helps clients live independently in their own homes. It is a permanent full-time position with two days per week from home.

Key responsibilities

• Provide effective and efficient care management for clients receiving individualised Home Care Packages
• Deliver administrative support and coordinate services for these clients
• Implement a person-centred, consumer-directed model to help clients achieve their goals and live independently at home

Required profile

• Tertiary qualification in Nursing, Allied Health or a related clinical field
• Current AHPRA registration
• Strong knowledge of the Aged Care Quality Standards, including Support at Home
• A person-centred clinical leader passionate about helping older people live well

What we offer

• Salary of $105,000 - $110,000 plus superannuation and salary sacrifice
• Salary packaging up to $18,550
