Client Support Partner (WA)

About the role

HenderCare is looking for a Client Support Partner to join its existing team in Western Australia. The role works with new and existing clients to support goal setting and deliver reablement strategies that promote independence, wellbeing and positive outcomes through person-centred care. The caseload is predominantly NDIS clients, with a smaller portfolio of aged care clients.

Key responsibilities

• Develop, review and maintain individualised care plans reflecting participant goals, preferences and funding eligibility.
• Work closely with clients, families and support networks so services match participants' goals.
• Conduct quarterly budget reviews and keep services within allocated budgets.
• Review service agreements regularly.
• Carry out risk assessments covering client health, home environment and personal safety.
• Help participants understand their approved services, funding classifications and rights.
• Promote HenderCare services to potential clients and referral sources.

Required profile

• Engaging, energetic and passionate, with attention to detail and the ability to learn and apply new processes.
• Ideally experience in case management in disability, mental health, healthcare, social or community services.
• Desirable: experience with Supported Independent Living (SIL) and managing complex client needs.
• Desirable: prior office or corporate experience and a sound knowledge of government and non-government health services.
• Right to work in Australia; pre-employment checks apply.
