Care Partner - Occupational Therapist

About the role

PartnerUP Talent is recruiting an Occupational Therapist for a newly created Care Partner position in Hobart. You will combine your OT expertise with care coordination, client relationships and practical problem-solving to support older people to stay independent, safe and living well at home. You will manage a caseload of Support at Home clients and become a key person in their care journey.

Key responsibilities

• Build meaningful relationships with clients and visit them in their homes to assess functional needs, independence and safety.
• Manage client budgets so that supports meet individual needs within the available package.
• Work closely with the wider team, nurses and support workers to achieve the best outcomes for clients.
• Complete home modifications and safety assessments to help clients stay safe and adapt their homes as their needs change.
• Take a holistic approach, understanding what matters to each client beyond their clinical needs.

Required profile

• Occupational Therapist with AHPRA registration.
• Ideally experience in aged care, community, home care, rehabilitation or a similar setting.
• New graduates or those whose experience comes from another clinical setting are welcome.
• Strong clinical reasoning, communication and problem-solving skills.
• Passion for building genuine relationships and for looking at the whole person.
• Team player who is practical, proactive and willing to think outside the box.
