Care Partner – Home Care Services

About the role

The Care Partner role is a permanent, full‑time position within HammondCare’s Home Care team, supporting clients in the Lismode and Kyogle areas. You will develop and maintain individualised care plans and ensure clients receive appropriate services.

Key responsibilities

• Deliver and continuously assess tailored Care Plans for each client.
• Manage care for existing clients, monitoring KPIs and ensuring plans align with funding and approvals.
• Provide day‑to‑day service delivery, supporting staff to understand client preferences.

Required profile

• Certificate IV in Aged Care with at least 2 years relevant experience.
• Knowledge of Support at Home, Commonwealth Home Support Programme and Aged Care Quality Standards.
• Current driver’s licence and willingness to travel between client sites.

Required skills

• Strong administrative and computer skills.
• Ability to operate effectively in a multi‑site, multidisciplinary environment.
