Care Manager - Residential Care Home

About the role

Anglicare Sydney is recruiting a Care Manager for its Kingswood Residential Care home, a permanent full-time onsite position between 9:00 am and 5:00 pm. The Care Manager leads and oversees clinical care delivery in the residential care community, working with the Residential Manager, Registered Nurses, Allied Health professionals and external healthcare providers to ensure safe, effective, person-centred care.

Key responsibilities

• Oversee high-quality clinical care and positive resident outcomes, and ensure care plans are developed, reviewed and updated
• Monitor resident health, wellbeing and clinical outcomes, and support multidisciplinary care planning with residents, families and professionals
• Maintain clinical benchmarks, infection control and quality standards
• Lead, mentor and support Registered Nurses and Allied Health team members, conduct performance reviews and support staff development
• Build relationships with residents and families, keeping them informed and supported
• Ensure care delivery meets organisational policies, professional standards and regulatory requirements, including documentation quality, funding compliance and clinical reporting
• Support clinical reviews, audits, quality improvement and workplace health and safety

Required profile

• AHPRA registration as a Registered Nurse (Division 1)
• 3-4 years of leadership experience in aged care or residential care as a Care Manager
• 5 years of clinical assessment, care planning and clinical governance knowledge as a registered nurse
• Experience leading multidisciplinary teams

Preferred skills

• Strong communication and stakeholder management skills
• Ability to manage competing priorities and make sound clinical decisions
• Computer literacy and confidence using clinical systems and documentation platforms
