About the role

Amplar Health Home Hospital has expanded its Transition Care Service with locations in Walkerville and Adelaide, offering a supportive sub-acute setting for public hospital patients who no longer need acute care. You will join a team bridging the gap between hospital and home, working closely with clinicians in a supervised environment.

Key responsibilities

Deliver exceptional patient care
Follow individualised care plans and work as part of a multidisciplinary team
Support patients with activities of daily living
Observe and report on patient changes

Required profile

Part-time availability (minimum 0.6 FTE) on a 7-day rotating roster covering morning, afternoon and night shifts
Certificate III in Health Services Assistance, Individual Support or similar relevant qualification
Ideally recent experience in a similar role
Team player with excellent communication and problem-solving skills
South Australian Working with Children Check and National Police Clearance (or willingness and eligibility to obtain them)
Willingness to provide proof of up-to-date vaccinations if successful
