Assessment Officer, HACC-PYP

About the role

The Home and Community Care Program for Younger People (HACC-PYP) is funded by the Victorian Government and supports eligible people under 65 (Aboriginal and Torres Strait Islander people under 50) who need help to stay independent and live safely at home. The Assessment Officer evaluates support needs and carries out in-home assessments for clients in the Macedon Ranges Shire, promoting consumer-directed care.

Key responsibilities

• Evaluate the support needs of younger people whose daily living activities are affected by disability, mental health, chronic condition or short-term health need (not eligible for NDIS)
• Undertake in-home holistic assessments in the Macedon Ranges Shire, focusing on enablement and quality of life
• Identify individual support needs, goals and care plans with clients and families
• Facilitate referrals to allied health and home care services
• Handle intake of incoming referrals within a set time frame
• Maintain client records and documentation in line with organisational and funding requirements
• Monitor targets, quality and compliance requirements to deliver good client outcomes

Required profile

• Tertiary qualification in Health, Mental Health, Social Work or Social Services, or commensurate experience
• Experience completing assessments and care plans in the health sector (desirable)
• Experience working with younger adults (desirable)
• Current Victorian driver's licence
• First Aid and CPR, or willingness to obtain them; Working with Children's Check, NDIS worker screening clearance, or willingness to obtain them
