Aged Care Comprehensive Assessor (Part-Time)

About the role

APM Group is hiring a Clinical Aged Care Assessor in Wallaroo, South Australia, on a permanent part-time basis of 4 days per week. The assessor identifies the needs of older adults and connects them with services that help them live independently with the highest quality of life. Daily travel across the service area is a core requirement, and the ideal candidate may live in Wallaroo, Moonta, Kadina, Ardrossan, Port Vincent, Stansbury or Minlaton.

Key responsibilities

• Conduct thorough aged care assessments to identify the needs of older adults.
• Facilitate referrals to essential services that support independent living.
• Promote the retention and maximisation of function and capability in daily living.
• Work closely with families and carers as key stakeholders.
• Ensure all assessments comply with Department of Health contractual requirements.

Required profile

• At least 1 year working with older Australians, including client assessments and/or service delivery.
• Tertiary education in a health-related discipline and professional registration with a relevant association (e.g. AHPRA).
• Current fully insured Australian driver's licence and ability to travel within the service area.
• Applicants must complete mandatory pre-employment checks, which may include background verification.
