Aged Care Comprehensive Assessor

About the role

APM is hiring a Clinical Aged Care Assessor in Port Augusta, South Australia, on a full-time basis. The assessor identifies the needs of older adults and connects them with services that help them live independently with the highest quality of life. Daily travel across the service area is a core requirement of the role.

Key responsibilities

• Conduct thorough aged care assessments to identify the needs of older adults
• Facilitate referrals to essential services supporting independent living
• Promote the retention and maximisation of function and capability in daily living
• Work closely with families and carers as key stakeholders
• Ensure assessments comply with Department of Health contractual requirements

Required profile

• At least 1 year working with older Australians, including client assessments and/or service delivery
• Tertiary education in a health-related discipline and professional registration with the relevant association (e.g. AHPRA)
• Current Australian driver's licence, a comprehensively insured vehicle and the ability to travel within the service area
• Proficiency in Word, Excel, Outlook and client management systems
• Openness to work in a multidisciplinary team; eligible disciplines include registered nurse, chiropractor, optometrist, speech pathologist, occupational therapist, physiotherapist, dietitian, podiatrist, paramedic, radiographer, dentist, pharmacist, medical practitioner, counsellor, psychologist, social worker, exercise physiologist, osteopath, Chinese medicine practitioner and art/music therapist
