Physiotherapist – Community and Home Care

About the role

Join the Lifecare Homecare team within the APM Group to deliver high‑quality physiotherapy services to clients in a multi‑disciplinary setting. You will work both in‑home and community environments, ensuring person‑centred care and optimal health outcomes.

Key responsibilities

• Provide assessments, treatment planning, implementation and evaluation tailored to individual client needs.
• Deliver physiotherapy interventions in clients' homes and collaborate with allied health professionals, GPs and other healthcare providers.
• Prepare comprehensive reports and treatment management plans meeting standards of NDIS, DVA, Medicare and other funding bodies.
• Build and maintain effective relationships with case managers, GPs, community centres and other referral sources.

Required profile

• Physiotherapy degree with general AHPRA registration.
• Full working rights in Australia and a clean driver’s licence with access to a vehicle and comprehensive car insurance.
• Current Working with Children Check and NDIS Workers Screening Clearance (or ability to obtain).
• Willingness to undergo a National Criminal History check.

Required skills

• Ability to work autonomously in the community and as part of a multi‑disciplinary team.
• Experience or interest in working with older adults and adult disability populations.
