Clinical Nurse Consultant – Cardiac (Hearts at Home)

About the role

St Vincent’s Health Australia is looking for an experienced Clinical Nurse Consultant (Cardiac) to join the innovative Hearts at Home program. This full‑time, remote position supports individuals living with heart failure, enabling early discharge and reducing avoidable hospital admissions.

Key responsibilities

• Provide evidence‑based clinical assessment, care planning, and intervention via virtual care platforms.
• Educate and coach clients in self‑management of heart failure.
• Coordinate care with hospital and primary‑care teams to ensure seamless transitions.
• Utilise remote monitoring technology to track patient progress and intervene when needed.
• Contribute to service development and continuous improvement initiatives.

Required profile

• At least 5 years post‑registration nursing experience.
• Minimum 3 years in cardiac care, chronic disease management, or critical care.
• Strong background in digital health and virtual care models.
• Demonstrated ability to work collaboratively with multidisciplinary teams.
