SIL House Team Leader - Enrolled Nurse

About the role

A client of Prime Recruitment is hiring experienced House Team Leaders to oversee Supported Independent Living (SIL) homes across Perth Metro and Port Hedland. Reporting to the Service Delivery Lead, the role combines clinical expertise, team leadership and operational management to deliver participant outcomes. It is a full-time permanent position.

Key responsibilities

• Lead and support a team of Support Workers
• Oversee daily house operations and service delivery
• Coordinate participant care and support services
• Maintain care plans, documentation and compliance requirements
• Manage medication processes and clinical governance activities
• Build strong relationships with participants, families and stakeholders
• Drive quality, safety and continuous improvement

Required profile

• Current AHPRA registration as an Enrolled Nurse (EN)
• Minimum 3 years' experience in disability, healthcare or community services
• Demonstrated leadership or team coordination experience
• Previous experience as a House Team Leader, Team Leader or Senior Support Worker
• SIL or residential disability experience
• Current driver's licence, First Aid, CPR and NDIS Worker Screening Clearance

Required skills

• Strong understanding of NDIS standards and person-centred care
• Care planning, documentation and stakeholder engagement
• Certificate IV or Diploma in Disability, Community Services or Leadership & Management
• Knowledge of Positive Behaviour Support, mental health or complex care
