Registered Nurse – Hybrid Home Care Role

About the role

This full‑time Registered Nurse position is a hybrid role based in Nundah, Queensland, combining in‑home client visits with office and remote work for care coordination and documentation.

Key responsibilities

• Conduct comprehensive clinical assessments and develop or review individualized care plans.
• Administer medications, perform wound care and other nursing procedures, and monitor client health status.
• Maintain accurate clinical documentation and contribute to risk assessments.
• Collaborate with clients, families, caregivers, and allied health professionals to ensure person‑centred care aligned with NDIS and aged‑care standards.
• Participate in case conferences, service planning and ensure compliance with clinical guidelines and regulatory requirements.

Required profile

• Current registration as an RN with AHPRA.
• Demonstrated experience or strong interest in community/home care, aged care, disability support or NDIS‑funded services.
• Ability to work autonomously, prioritize competing demands and adapt to changing client needs.
• Valid driver’s licence, reliable transport and willingness to obtain required clearances (Police Check, NDIS Worker Screening).
