Permanent Part-Time Clinical Care Partner

About the role

Allawah Community Care, a community care provider delivering services under the Commonwealth Home Support Programme (CHSP) and the Support at Home (SAH) program, is hiring a permanent part-time Clinical Care Partner. The role coordinates care for clients with higher-level and complex needs, provides clinical oversight and support to the Community Care team and delivers direct nursing care where required. It involves clinical assessments, care plan development and review, and supporting non-clinical Care Partners, working a minimum of two days per week.

Key responsibilities

• Conduct nursing assessments of clients' health and support needs and provide direct nursing care in clients' homes
• Develop, implement and regularly review individualised care plans with clients, families and health professionals
• Monitor clients' health status and adapt care plans to changing needs
• Support and train non-clinical care staff and care partners, and manage client budgets in line with program guidelines
• Identify, manage and escalate clinical risks and changes in health or care needs
• Contribute to clinical governance, incident and risk management, and continuous quality improvement
• Educate clients and families about care plans, service options and their rights
• Maintain accurate client records and comply with regulatory standards

Required profile

• Bachelor of Nursing (essential)
• Current AHPRA registered nurse registration (essential)
• Valid Australian driver's licence and access to a reliable vehicle
• Initial and ongoing National Police Clearance (essential)
• Not listed on the Aged Care Banning Order register (essential)
• Experience in aged care, community nursing or related clinical roles (desirable)

Required skills

• Understanding of aged care service delivery models and Aged Care Quality Standards
• Ability to provide direct nursing care and clinical leadership
• Culturally safe, trauma-informed care approach
