NDIS Support Coordinator - Mackay

About the role

Community Solutions Group is looking for an NDIS Support Coordinator for a full-time permanent position in Mackay. The role helps NDIS participants achieve greater independence by assessing their individual needs, identifying the services and supports needed to reach their goals, coordinating their supports and addressing barriers through action plans.

Key responsibilities

• Build strong relationships with participants, their families and external service providers
• Drive to visit participants as required
• Help participants negotiate with providers about what they will offer and understand the costs they must allocate from their NDIS budget
• Work with billable hours and targets
• Ensure service agreements are completed
• Take a holistic approach to capacity building and help participants break down and address their barriers

Required profile

• Previous experience working with, and a thorough understanding of, the NDIS and its plans and budgets
• Experience with billing targets and case noting
• People-oriented, preferably with disability sector experience, able to motivate and build rapport with participants, families and providers
• Business mindset and customer service focus
• Excellent time management, prioritisation and organisation skills
