Community Nurse – RN/EN

About the role

Focus Care is seeking a passionate Registered or Enrolled Nurse to join our Clinical Team. Reporting to the Quality and Clinical Manager, you will provide in‑home care that enables clients to live their best lives while also supporting office‑based coordination.

Key responsibilities

• Conduct clinical assessments of clients in their homes under supervision.
• Identify care needs and take appropriate action.
• Liaise with families, GPs, and allied health providers to coordinate care.
• Coach clients on self‑care and conduct training for Support Workers.
• Provide nursing care for clients with complex needs.
• Review clinical incidents and report recommendations to management.

Required profile

• Current registration with AHPRA as a Registered or Enrolled Nurse.
• Experience in health, community, aged‑care or disability sectors.
• Ability to work autonomously and manage time effectively.
• Strong communication and collaboration skills.
• Valid driver’s licence, current First Aid/CPR certificate and National Police Check.
