Clinical Pharmacist (Grade 1) – Part‑time

About the role

The Clinical Pharmacist Grade 1 provides clinical pharmacy and operational support across McKellar Centre, focusing on sub‑acute services such as rehabilitation, palliative care and geriatric pharmacy. Working 9:00 am‑2:30 pm weekdays, the role balances patient‑centred care with a supportive multidisciplinary team.

Key responsibilities

• Deliver clinical pharmacy services including medication reconciliation, supply, medicines information, patient counselling and discharge planning for sub‑acute and inpatient services.
• Support safe, cost‑effective medication use through formulary compliance, antimicrobial stewardship, restricted medicine management and medication safety activities.
• Collaborate with medical, nursing, allied health and pharmacy teams to optimise patient care and ensure timely medication management.
• Contribute to education, service delivery, quality improvement and professional development while supporting students, interns and pharmacy technicians.

Required profile

• Registered Pharmacist with AHPRA registration and hospital or clinical pharmacy experience, preferably in sub‑acute or inpatient settings.
• Strong knowledge of pharmacotherapy and ability to support safe, evidence‑based medication management.
• Excellent communication and teamwork skills for collaborative multidisciplinary work.
• Commitment to ongoing professional development, reflective practice and interest in education, quality improvement and patient‑centred care.
