Clinical Partner – Nursing (Home Care)

About the role

Join HammondCare as a Clinical Partner – Registered Nurse in the South Coast Region, based in Merimbula. You will work within the Home Care Clinical Care & Practice Team, providing person‑centred assessments, care planning and interventions to older adults living at home.

Key responsibilities

• Assess functional care needs of clients, set care goals and deliver tailored treatment.
• Conduct home visits and facilitate group activities for clients receiving Support at Home, CHSP services or residing in Seniors Living units.
• Collaborate with a multidisciplinary team to ensure continuous quality improvement and compliance with AHPRA standards.

Required profile

• Registered Nurse with current AHPRA registration and experience in aged care, community, home‑based or rehabilitation settings.
• Strong communication skills, ability to plan and manage time effectively, and work independently within a team.
• Reliable vehicle, valid driver’s licence and safe driving record (travel costs reimbursed).
