Clinical Care Coordinator - Residential Aged Care

About the role

Calvary Health Care is recruiting an experienced Clinical Care Coordinator for its facility in The Mariner, South West Rocks, NSW. This is a full-time permanent position. The coordinator leads the care team in consultation with the Home Manager, ensuring compliance with legislative and statutory requirements so that residents receive a high standard of care and a quality lifestyle.

Key responsibilities

• Coordinate the delivery of quality clinical services in line with a resident-centred model of care
• Maintain current knowledge of Aged Care Funding models
• Manage and provide efficient, effective and compassionate care planning and assessment
• Coordinate the availability of medical stock and other health-related supplies to ensure sufficient and adequate supplies

Required profile

• Registered Nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
• Demonstrated knowledge of the legislation governing aged care
• Commitment to contemporary aged care nursing practice and to Calvary's mission, vision and values
• Alignment with a resident-focused philosophy supporting dignity, independence and informed decision-making
• Ability to support a multi-disciplinary team and strong oral and written communication skills
