Clinical Care Coordinator - Coastal Relocation

About the role

A residential aged care team in a coastal town on the NSW Mid North Coast, about five hours north of Sydney, is looking for an experienced Clinical Care Coordinator. You will work alongside the Manager to coordinate clinical care, support the care team and ensure residents receive safe, high-quality, person-centred care. Relocation assistance is offered to candidates moving to this community.

Key responsibilities

• Coordinate clinical care and support the care team
• Carry out clinical assessments and care planning
• Support staff and maintain compliance
• Promote positive resident outcomes

Required profile

• Current AHPRA registration as a Registered Nurse
• Experience in residential aged care, preferably in a clinical role
• Strong clinical leadership and communication skills
• Sound understanding of aged care standards and legislation
• Interest in relocating to a coastal community
