Aboriginal Care Coordinator (Renal Health)

About the role

Kimberley Aboriginal Medical Services (KAMS), the peak regional Aboriginal Community Controlled Health Service in the Kimberley region of Western Australia, is recruiting an Aboriginal Care Coordinator to work in Derby on a full-time basis. The role establishes and implements care coordination services for Aboriginal patients with renal conditions, providing information and education on renal health, engaging patients in outreach settings and advocating for their rights within a multidisciplinary team.

Key responsibilities

• Develop strategies to improve access to primary healthcare for Aboriginal and Torres Strait Islander people
• Facilitate culturally sensitive health care delivery
• Work collaboratively with local Aboriginal Medical Services
• Provide information and education on renal health and engage patients in outreach
• Advocate for patient rights within a multidisciplinary team framework

Required profile

• Aboriginal or Torres Strait Islander descent
• Experience working with Indigenous communities from diverse backgrounds
• Strong organisational skills and attention to detail
• Excellent communication and negotiation abilities
• Commitment to confidentiality and cultural sensitivity
• Current 'C' class manual driver's licence
• Desirable: health setting experience, mentorship capacity for Aboriginal staff, computer skills and ability to travel within the Kimberley region

What we offer

• Salary of $85,126 per annum pro rata
• Accommodation allowance of $7,800 per annum pro rata
• Annual airfare of $1,800 per annum pro rata
• Opportunity for salary sacrifice up to $30,000 per annum
