Support at Home Case Manager

About the role

Calvary Home Care Adelaide is recruiting a full-time Support at Home Case Manager based in its Adelaide CBD office. The role coordinates and delivers person-centred care that helps older Australians live independently at home, combining office work with regular community visits, supported by access to company vehicles. Standard business hours apply.

Key responsibilities

• Manage a caseload of clients, including assessments, care planning and regular reviews to support independent living
• Coordinate and monitor in-home services, working closely with clients, families and service providers
• Ensure compliance with Support at Home guidelines while keeping accurate documentation and delivering person-centred care

Required profile

• Understanding of the new Aged Care Act and the Support at Home program
• Full driver's licence
• Certificate III or IV in Aged Care, Disability or Community Service, Diploma of Community Services (Case Management), or a Bachelor's degree in Social Work
• Experience in case management and coordination
