Social Worker - Ambulatory Aged Care

About the role

Goulburn Valley Health is recruiting a Social Worker for its Ambulatory Aged Care Program (AACP), which includes the Transition Care Program, the Restorative Care Program and GEM In The Home. The position serves AACP clients, their families and significant others in a community setting, working as an integral member of the multidisciplinary team. It is a permanent part-time role of 64 hours per fortnight, based at New Dookie Road.

Key responsibilities

• Provide social work services to AACP clients, their families and significant others in a community setting
• Plan and implement high-quality social work interventions
• Focus on continuous improvement and evaluate the quality and outcomes of interventions
• Communicate effectively with internal departments and external services to achieve best practice outcomes

Required profile

• Approved tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
• Demonstrated skills in assessment and intervention for patients with diverse and complex psychosocial circumstances
• Understanding of person-centred care in relation to adjustment to illness, disability or lifestyle changes
• Ability to work independently and as an active member of an interdisciplinary team

Required skills

• Excellent interpersonal, written and verbal communication skills
• Ability to build and maintain professional relationships with internal departments, community agencies and services
