Regional Clinical Pharmacist

About the role

The WA Country Health Service South-West is seeking experienced clinical pharmacists to join the Bunbury team. This position focuses on delivering safe, rational, and cost‑effective medication therapy for hospital patients across the South West region.

Key responsibilities

• Perform medication reconciliation for admitted patients and maintain comprehensive medication profiles.
• Dispense medications and provide drug information to nursing and medical staff.
• Make independent therapeutic decisions to optimise patient outcomes.
• Collaborate with multidisciplinary teams to support quality use of medicines.
• Adhere to Pharmacy Board of Australia standards, codes, and WACHS policies.

Required profile

• Demonstrated ability to deliver evidence‑based clinical pharmacy services in a hospital setting.
• Strong communication skills for interacting with patients, carers, and healthcare teams.
• Commitment to continuous professional development and adherence to professional standards.
