Disability Support Worker - Community

About the role

The Community Support Worker role assists NDIS‑funded clients to achieve greater independence and community participation. Working across the Midland area, you will support people with disability or psychosocial needs in daily living activities, social outings, and health appointments.

Key responsibilities

• Accompany clients to social activities, shopping trips, and GP or specialist appointments.
• Provide personal care at clients' homes, including toileting, showering, dressing and hygiene support.
• Perform light domestic duties, meal preparation and eating assistance.
• Administer medication safely and according to client plans.
• Liaise with families, carers and other representatives as required.

Required profile

• Commitment to person‑centred care that respects client choice and goals.
• Ability to build and maintain meaningful relationships with clients.
• Valid driver’s licence, reliable vehicle and willingness to travel within the Midland area.
• Proof of working rights (passport or visa) and recent National Police Clearance.
• Current NDIS Worker Screening Check and Working With Children Check (WA).
• Previous experience in disability or health support or a relevant Certificate III (desirable but not essential).
