Disability Support - Support Leader

About the role

Leap Support Services Tasmania is looking for an experienced Support Leader to lead day-to-day support for participants and the Support Workers on its sites. The role is hands-on and on-the-ground, based between Wynyard and Latrobe, and is part-time.

Key responsibilities

• Provide day-to-day, person-centred support to participants and give leadership and coaching to Support Workers across allocated sites
• Embed the organisation's values and person-centred practice in the team's work
• Keep care plans, behaviour support plans and documentation current, understood and followed
• Maintain a visible, hands-on presence across sites and varying shifts
• Coordinate participant appointments, keep records accurate and communicate changes to Support Workers
• Own accurate, timely reporting, including medication management and end-of-month documentation
• Identify and escalate risks, incidents and wellbeing concerns, and seek input from the Supervisor
• Build trusted relationships with participants and their families, and contribute to the after-hours on-call roster

Required profile

• Certificate IV in Disability, Community Services or equivalent (or working towards)
• Demonstrated experience in disability support with complex needs
• Current driver's licence and ability to obtain (or hold) a NDIS Worker Screening Check, Working with Children Check and vulnerable people registration
• Ability to manage competing priorities and stay calm under pressure
