Dementia Support Coach

About the role

Dementia Support Australia (DSA), a national dementia support service funded by the Australian Government and led by HammondCare, is hiring a Dementia Support Coach in Osborne Park. The role sits within the Hospital to Aged Care Dementia Support Program (HACDSP) and uses coaching skills and aged care experience to give hands-on, evidence-based support and advice to clients, carers and care staff. It is a fixed-term contract until June 2028.

Key responsibilities

• Work with peers to help aged care staff and carers develop the skills, expertise and applied knowledge needed to support people living with dementia.
• Support the implementation of Dementia Support Australia plans on the ground using modelling and coaching techniques, with guidance from Team Leaders and Dementia Consultants when needed.
• Manage and coordinate brokerage items with clients and their carers to improve quality of life.
• Act as a liaison and trusted partner with staff and carers, helping them build confidence in dementia care practice.

Required profile

• Qualification in a relevant field such as Allied Health Assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, or Cert III or IV in Individual Support / Leisure and Health; similar qualifications highly desirable.
• Current driver's licence and safe driving record.
• Willingness to travel and work intrastate and interstate.
