Community Services Coordinator - NDIS

About the role

St Jude's Health Care Group, a provider of aged care, allied health and disability services, is hiring a Community Services Coordinator for its new West Perth office. The role ensures that person-centred services meet client needs through regular contact with clients, their families and other stakeholders, while managing staff performance, using funds responsibly and meeting budget requirements. It is a full-time position, Monday to Friday 9am to 5pm.

Key responsibilities

• Work closely with clients so that their funds and services are used and delivered as effectively as possible
• Check that support plans are followed and that high-quality progress notes are kept
• Manage and support a team of support workers
• Ensure services are compliant with standards and that risks are managed effectively

Required profile

• Proven track record of quality services in person-centred or community programs, ideally including participants with complex mental health issues or challenging behaviour
• Understanding of the NDIS funding model and reporting requirements, and ability to use it to resolve complaints with stakeholders
• Proactive self-starter who leads performance appraisals and identifies growth areas in a continuous improvement plan
• Relevant qualifications or certifications, or significant experience within the NDIS framework
• Willingness to obtain an NDIS screening clearance, a WA driver's licence and a reliable vehicle
