Clinical Pharmacist - Slade Pharmacy Richmond

About the role

Slade Pharmacy Richmond, part of Icon Oncology Group, is seeking a Grade 2 Clinical Pharmacist to join its team within Epworth Hospital. The role sits in a dedicated private hospital environment and offers the chance to upskill in general medicine, cardiology, oncology, emergency medicine, intensive care and surgical specialties. The service runs Monday to Friday, 8.00am to 4.30pm.

Key responsibilities

• Deliver clinical pharmacy services to allocated wards, including medication chart reviews, reconciliation and documentation of allergies and adverse drug reactions.
• Give evidence-based advice to medical, nursing and allied health staff on the safe use of medicines.
• Counsel patients and carers on prescribed therapies and promote adherence.
• Take part in clinical rounds and multidisciplinary team meetings.
• Support the safe and ethical use of medicines, including investigational and compassionate supply medications.
• Resolve medication-related problems with prescribers and contribute to medication safety, quality improvement and audits.
• Assist in training and supervising pharmacy interns, students and technicians.

Required profile

• General registration as a Pharmacist with AHPRA.
• Bachelor of Pharmacy or equivalent tertiary qualification.
• Previous experience dispensing medication in an Australian or New Zealand community or hospital pharmacy.
• Desirable: hospital or clinical pharmacy experience, mentoring experience, involvement in quality improvement, audits or clinical trials, and membership of AdPha, COSA or ISOPP.
