Clinical Care Coordinator - St Vincent's Care at Home

About the role

St Vincent's Health Australia is expanding its National Care Coordination Centre as part of its Care at Home portfolio and is recruiting experienced Registered Nurses to join during the service's implementation phase. The Clinical Care Coordinator coordinates patient care journeys from referral through to discharge, working with multidisciplinary teams, hospitals, referrers and community providers. Permanent full-time and part-time positions are available in Kangaroo Point, Brisbane, on a 7-day rotating roster.

Key responsibilities

• Assess referrals and coordinate patient journeys from referral through to discharge
• Coordinate treatment pathways, monitor patient progress and manage clinical escalations
• Support virtual care, remote monitoring and care navigation services
• Contribute to service improvement initiatives and the development of new care models

Required profile

• Current unrestricted AHPRA registration as a Registered Nurse
• At least 5 years' clinical nursing experience in acute, community, chronic disease, care coordination or sub-acute care
• Demonstrated experience coordinating complex patient care journeys
• Experience in patient-centred models of care
• Highly regarded: postgraduate clinical qualifications, virtual care or telehealth experience, care coordination or patient navigation experience, remote patient monitoring exposure, and experience implementing new services

• Well-developed digital literacy and use of clinical information systems
