Clinical Care Coordinator - Ambulatory Aged Care Programs

About the role

Goulburn Valley Health is recruiting a Clinical Care Coordinator for its Ambulatory Aged Care Programs (AACP) in the West Hume Region. The AACP consists of three programs: the Transition Care Program, which supports older people after hospital discharge in a non-hospital setting; the Restorative Care Program, which serves inpatients of the health service; and GEM in The Home, a person-focused service aimed at improving and maintaining functional capacity and independence. This is a permanent part-time position of 64 hours per fortnight.

Key responsibilities

• Provide care coordination, advocacy and support for AACP clients and their caregivers in the West Hume Region.
• Case manage clients and coordinate services within a care coordination model.
• Conduct comprehensive, person-centred assessments and develop and monitor individual goal-oriented care plans, within service and organisational requirements including budget and regulatory requirements.
• Work collaboratively as an individual practitioner within an interdisciplinary team, building and maintaining effective relationships.

Required profile

• Current registration with the Australian Health Practitioner Regulation Agency in the relevant discipline, or eligibility for membership of the relevant professional peak body.
• Tertiary qualification in nursing or an allied health discipline, with eligibility for professional body membership.
• At least three years of clinical experience and the ability to practice across acute, sub-acute aged care and community sectors.
• Demonstrated ability to assess clients and coordinate services.
