Case Manager - Aged Care

About the role

Calvary Home Care in Darwin is looking for a Case Manager to manage a caseload of clients funded under CHSP/HCP. The role covers maintaining budgets, onboarding new clients and managing care requirements so clients can stay independent. Calvary is described as one of Australia's leading health care providers, with home care, residential aged care and public and private hospitals.

Key responsibilities

• Manage a small team of various roles
• Manage a caseload of clients under CHSP/HCP approved funding
• Maintain budgets and onboard growth of new clients
• Manage care requirements to maintain client independence
• Understand the Aged Care Standards
• Liaise with stakeholders and internal networks

Required profile

• Evidence of leading a fast-paced team
• Evidence of leadership in a Case Management or senior case manager role
• Demonstrated experience in the aged, community or disability sector, particularly with clients with complex needs
• Relevant tertiary qualification in the community sector or Cert4 in leadership (desirable)
• Full driver's licence and own reliable registered and insured vehicle
• NDIS clearance (or willingness to obtain one) and police clearance within the last 6 months (or willingness to obtain one)
