Assistant in Nursing - Transitional Aged Care Program

About the role

Hunter New England Health is recruiting an Assistant in Nursing for its Transitional Aged Care Program (TACP) at Wattle Grove Cottage, Wallsend Health Campus. The team supports older people as they move from hospital back to home, working alongside nurses and Allied Health professionals. The position is permanent part-time, 24 hours per week, with pay of $32.85-$36.04 per hour plus superannuation and salary packaging.

Key responsibilities

• Assist clients with personal care and activities of daily living.
• Support home-based and community exercise programs and therapeutic activities prescribed by Allied Health professionals.
• Assist with mobility, safe manual handling and falls prevention.
• Provide transport to appointments and help clients access community services.
• Offer encouragement and psychosocial support to help clients build confidence and reach their rehabilitation goals.
• Follow documented care plans and work with the nursing and Allied Health team to deliver safe, coordinated care.

• Current driver's licence and ability to travel within the community as required.
• To be eligible for permanent employment, you must be an Australian citizen, a permanent resident of Australia, or a New Zealand citizen with a current passport; visa sponsorship is not available.

Required skills

• Understanding of safe manual handling, mobility assistance, falls prevention and risk management.
• Ability to carry out care tasks and prescribed exercise programs according to documented care plans.
