Local Area Coordinator – South East Sydney

About the role

The Local Area Coordinator (LAC) will support people living with disability or chronic health conditions across the South East Sydney region. Working within Latrobe Community Health Service, you will help participants navigate the NDIS, develop personalised plans and connect them with community resources.

Key responsibilities

• Support participants to access and navigate the National Disability Insurance Scheme (NDIS).
• Collaborate with individuals, families and carers to develop plans aligned with participants’ goals.
• Build and maintain connections with community and mainstream support services.
• Strengthen participant independence and capacity through tailored interventions.
• Manage a diverse caseload while meeting performance and KPI expectations.

Required profile

• Strong empathy and excellent listening skills.
• Organisational and planning abilities.
• Effective verbal and written communication.
• Ability to work to KPIs in a fast‑paced environment.
• Current NDIS Workers Screening Check.
• Current Working with Children Check.
