Community Nurse – Home Care (EN/RN)

About the role

We are seeking a passionate Registered or Enrolled Nurse to join our Clinical Team. Based in our office, you will travel to clients’ homes to assess needs, deliver preventative care, and support them in living their best lives.

Key responsibilities

• Conduct clinical assessments of clients in their homes under supervision.
• Identify care needs and take appropriate action.
• Liaise with families, GPs, and allied health providers to coordinate care.
• Provide coaching for client self‑care and conduct training for Support Workers.
• Deliver nursing care to clients with complex needs.
• Review clinical incidents and report outcomes and recommendations.

Required profile

• Current registration with AHPRA as a Registered or Enrolled Nurse.
• Experience in health, community, aged‑care or disability sectors.
• Ability to work autonomously and manage time effectively.
• Strong communication skills for multidisciplinary collaboration.
• Valid driver’s licence, current First Aid/CPR certificate and National Police Check.
