Case Manager - Permanency Support Program

About the role

CatholicCare is hiring a Case Manager for its Permanency Support Program (PSP) Team in Campbelltown. The role assesses client needs, develops personalised care plans and coordinates services to support children, their families and carers toward safety, wellbeing and stable care outcomes. It is a permanent, full-time position of 38 hours per week.

Required profile

• Tertiary qualification in Social Work, Welfare, Behavioural or Social Sciences, or another relevant discipline.
• Proven case management experience, with a focus on Out of Home Care (OOHC) and permanency planning.
• Strong organisational and time-management skills.
• Exceptional interpersonal and communication abilities.
• Ability to work independently and as part of a team.
• Paid NSW Working with Children Check (WWCC).
