Community Registered Nurse – Home Care (Wed‑Fri)

About the role

Silverchain is seeking a Registered Nurse to provide high‑quality, in‑home care for older Australians. You will work independently across the Adelaide metropolitan area on Wednesdays, Thursdays and optionally Fridays, delivering essential clinical services directly to clients’ homes.

Key responsibilities

• Perform complex wound care, catheter (IDC/SPC) insertion and management, and stoma/continence support.
• Manage chronic disease conditions through advanced clinical assessments and medication administration.
• Travel to client residences in Far‑Noarlunga, Hackham, the City and Western Suburbs.
• Document care accurately and communicate findings with families and multidisciplinary teams.
• Provide education and support to clients to promote safety and independence.

Required profile

• Degree or postgraduate qualification in nursing.
• Minimum 3 years post‑graduate experience in general or community nursing.
• Current AHPRA registration with no notations.
• Genuine interest in aged‑care and community settings (experience advantageous).
• Compassionate, enthusiastic attitude with strong interpersonal communication.
• Valid Australian driver’s licence.
